Urgent help needed- cant add fields

Hello everyone,

Pl help..the form is wrking fone..sending attachments..except that i am trying to add more text fields..but it is not showing in the results emailed to me.

i have tried all options, text variable names are all ok..but cant get them to be shown in email.

pl paste a sample code for extra text fileds i can add..i.e age, sex,country all simple text fields.
