quiestion about phmailer

Hi ,
i tried php mailer it works great and want to use this phmailer
but....
i want to use this form for a sportclub
that people can ask some info
but also want to use this phmailer for
when we have a couple tournement

does or can anyone help me with the follow thingy :

i want to add in this phmailer some more options like :
some extra fields like :

a second name ,
and a phonenumber field

not neccessary as requierment
