Problem w/ multiple attachments making the whole form repeat

First of all, great script!

I'm having some trouble working with multiple attachments. If I set the attachment limit to more than one, and for some reason it is repeating the entire form that number of times.
$allowattach="3";

You will notice that the form is repeated three times - I set this to five and it repeated itself five times.

In addition to this problem, I assume that the field names for each attachment should be unique?
i.e.
<input name="attachment[]2" type="file" size="30" />
<input name="attachment[]2" type="file" size="30" />
<input name="attachment[]3" type="file" size="30" />

Is this correct?

The repeating form is very strange - any suggestions?

Never mind - it's fixed!

Can you tell me how you got multiple attachments to work? I can't seem to figure it out.

All you have to do is adjust the value in the php file to how many attachments you want to allow. if you have done that and still have problems, contact your website host and ask them if they have any restrictions on it.
