1.) Sample.ifl
2.) Sample.npz
3.) Sample.WAC
4.) Sample.str
5.) SS0000.jpj [screen shot of the map/combat area]
6.) readme.txt

all into one zip file.
[don't know what the zip-file size limit is]

readme.txt should have in it:
map maker; any general info like for sample you use another type of mod to make this map with, and any spec/details you like to add to readme.txt so other know were they can get hold of you if they found a error in the map or if the map crash they can ask you about it.

maybe a good ID to put a link to the page if you use a mod to make the map with so they can get the mod or a custom-terrain for it to download them to use the map without error to it.
